AACN Conference 2019

Below are links to evaluation forms for each workshop presented at the 17th Annual Conference of the AACN (June 5-8, 2019 in Chicago, IL). Attendees should complete one evaluation form for each workshop attended. After submitting an evaluation form, attendees will receive a CE certificate by email. Each certificate may also be downloaded or printed from this website.

A handout with instructions is included in each conference attendee’s registration materials. Click here for an extra copy.

Please note the following required prerequisites for completing an evaluation form:

  1. You must have registered for the conference.
  2. You should have received an email indicating that the evaluation form was ready to be completed. These emails will generally be sent within 48 hours after the conclusion of a workshop.
  3. You must be logged into this website.

If any of these prerequisites have not been met, then you will not be able to access the evaluation form. If all required prerequisites have been met but you are still having an issue, then you may contact us for assistance.
